2020 Lafayette Parish Medical Society Alliance Scholarship for Bachelor

Eligibility Criteria

• Applicant must be a resident of Lafayette Parish in the state of Louisiana for at least seven (7) years prior to applying for this scholarship
• Applicant must be a Louisiana resident for at least 12 months prior to registering as a medical student
• Applicant must be accepted into or enrolled full time in an accredited United States or International medical school

Application Process

1. A completed application form
2. Proof of current enrollment. (If you are a first-year medical student, please provide proof of acceptance.)
3. A typed one-page essay addressing the following: reasons for pursuing a medical career, personal goals and plans for your future within the profession
4. Two reference letters, from persons other than family members, documenting the applicant's integrity, interpersonal skills, and potential as a physician
5. A 2-3 minute video of yourself answering questions provided in the scholarship application below
6. Scholarship deadline is Friday, May 29, 2020
